  • In an effort to ensure all students at Curtis Elementary are provided with needed supports, we have created the form, "Parent/Guardian Request for Student Assistance". 
    Parents and Guardians, Please fill out the form whenever you feel support is needed. The Tier 2 team will review the form, make contact with the parent/guardian, and take one of the following steps:
      • come up with strategies, interventions, and/or a support system for the particular situation
      • provide resources to assist with the situation
      • refer student/parent to other resources to assist with the situation

      There are directions and an explanation of our Parent/Guardian Request for Student Assistance form on page 13 of our Parent/Student Handbook.
