Attached below are links to the parent volunteer forms, along with information about the process. 

Click on the link below, complete the application, and submit it to the school for Principal’s approval.  Applicants need to also complete a Tuberculosis (TB) test (administered within the last 60 days).  Once the application and TB test are complete, call Personnel Services at (909) 820-7700 ext. 2400 to schedule a fingerprint appointment.

Los solicitantes necesitarán una prueba de Tuberculosis (TB) (administrada dentro de los últimos 60 días). Ya que la solicitud y la prueba de TB estén completas, llame a Servicios de Personal al (909) 820-7700 ext. 2400 para hacer una cita de huellas dactilares.
